- Login to your account at www.ncbtmb.org
- Click Additional Forms from the Quick Links menu
- Complete the Approved Provider Organization Name Change Form
- You will need to submit legal documentation showing the name was changed
- Click Submit
NOTE: Many states look at NCBTMB’s website to ensure certificates are legitimate. Changing the name of your organization without adding your new name, along with “formally known as…” may cause a licensee’s certificate to be denied. Please ensure clarity of names and past names on all certificates.
