Assigned School Code Request (pdf)
To receive an Assigned School (AS) number, complete this interactive form, print, enclose required documents and send to NCBTMB (address provided on form). This number must be included on your students’ applications to sit for an NCBTMB exam.

Verification of Education (pdf)
This form can be used in lieu of a transcript (only if applicable), but must be signed and dated by a school official. A school seal is recommended.

School Pass Rate Request (pdf)
Complete this interactive form to request official reports of the performance rate of your school. Then print it, enclose fee and send to NCBTMB’s testing service, Pearson VUE (address provided on form).

Mailing List Request (pdf)
Complete this interactive form if you are interested in purchasing a mailing list of NCBTMB practitioners. Then print it and send it to NCBTMB (address provided on form).

Volunteer Application: State School Outreach Representative (pdf)
If you are a faculty member at a massage school, serve as an administrator, or teach continuing education and are interested in serving as a state school outreach representative, complete this application. Applicants who interview and are selected will serve as a representative for fifteen (15) months. This is an exciting opportunity to help support massage schools, act as a role model for future volunteers, and reach out to future peers in the profession.
