NEW NCBTMB Approved Provider Application and Fee Schedule
Effective 1/1/09
NCBTMB has released a new Approved Provider Renewal Application and Reference Guide to make the renewal process quicker and easier. As of January 1, 2009, previous versions of the application are no longer being accepted.
In addition, new application fees also went in to effect January 1, 2009 -- $175 for individual providers and $400 for organizational providers.
