Recertification Application
To apply for recertification online, you must have a valid Visa or Mastercard. Or you can download the Recertification Application and the Recertification Reference Guide, which contain all the detailed information you need to recertify.
Official Score Transfer Request: Tested Before 1/1/2008 (pdf)
If you tested before 1/1/2008 and need a copy of your official score sent to an employee or state licensure board, complete this interactive form. Then print it and send it to NCBTMB (address provided on form).
Official Score Transfer Request: Tested After 1/1/2008 (pdf)
If you tested after 1/1/2008 and need a copy of your official score sent to an employee or state licensure board, complete this interactive form. Then print it and send it to the testing center, Pearson VUE (address provided on form). For the status of your Score Transfer, email NCBTMB@pearson.com. Please allow 7-10 business days before requesting an update.
