Recertification Application
To apply for recertification online, you must have a valid Visa or Mastercard. Or you can download the Recertification Application and the Recertification Reference Guide, which contain all the detailed information you need to recertify.
Official Score Request Form
If you need a copy of your official score sent to an employee or state licensure board, complete this interactive form.
Duplicate Certificate Request (pdf)
Complete this interactive form if you would like a duplicate copy of your certificate – or if you have an official name change.
NESL Certificate Conversion (pdf)
If you passed the NESL for state licensure, but now wish to convert your NESL status to national certification, click here for a pdf of the conversion application.
